Qu’est-ce que l’aphasie ?
« L’aphasie est un trouble du langage oral et/ou écrit qui résulte d’une lésion cérébrale des zones impliquées dans le langage », explique le Dr Bertrand Glize, médecin de rééducation au CHU de Bordeaux, et expert de cette pathologie. Cette maladie se traduit par une perte de la capacité à communiquer partielle ou totale pouvant entraîner d’autres troubles associés (écriture, lecture). Souvent invalidante, l’aphasie affecte la vie familiale, sociale et professionnelle des personnes touchées.
Quelle est la fréquence de cette pathologie ?
Actuellement, la Fédération Nationale des Aphasiques de France (FNAF) estime que 300 000 personnes seraient aphasiques en France. Les troubles qui y sont associés peuvent se manifester à des degrés divers chez les personnes touchées.
Quels sont les symptômes de l’aphasie ?
Dans son quotidien, une personne aphasique souffre de difficultés à s’exprimer, comprendre, lire ou écrire. Il en existe plusieurs formes qui peuvent se manifester par :
Troubles de la production du langage : il s’agit de difficultés à produire les mots correctement, associée à une diminution de la fluence verbale, c’est-à-dire au « débit » des mots exprimés. On parle d’aphasie non-fluente. Ces atteintes vont du « manque du mot » qui caractérise les anomies à des troubles plus sévères. L’aphasie motrice ou l’aphasie de Broca, en référence historique à Paul Broca qui l’a décrite, est un trouble caractéristique de la production du langage n’affectant que peu la compréhension.
Troubles de la compréhension, associés ou non aux troubles du langage : L’aphasie de Wernicke, en référence à Karl Wernicke qui l’a décrite, est caractérisée par des troubles importants de la compréhension. Si la fluence est préservée, les productions correspondent à un jargon, le patient n’ayant pas la possibilité de critiquer et comprendre ce qu’il dit.
Zones du cerveau dont la perturbation provoque l’aphasie de Broca ou de Wernicke
Ces symptômes sont présents au niveau du langage oral et/ou écrit, pas de façon proportionnelle.
Outre l’aphasie de Broca et l’aphasie de Wernicke qui sont les plus connues, ces différences de proportionnalité ont amené les spécialistes à décrire d’autres formes d’aphasies. De manière non exhaustive, on peut citer :
l’aphasie de conduction : discours fluent, entrecoupé d’hésitations et de paraphasies. La compréhension est généralement bonne.
l’aphasie globale : L’expression du patient est quasi nulle. Les troubles de la compréhension sont également importants.
Enfin, un autre type d’aphasie à évolution progressive correspond à une atteinte neurodégénérative : l’aphasie primaire progressive.
Quelles sont ses causes ?
« Aphasie » vient du grec « phasis » (parole) et signifie « absence de parole ». Ce trouble est le symptôme d’une lésion cérébrale pouvant provenir de multiples causes. Il peut s’agir notamment :
D’un accident vasculaire cérébral (AVC) ; qui en la cause la principale. « On estime que 20 à 30 % des personnes victimes d’un AVC développent une aphasie par la suite », indique le Dr Bertrand Glize.
D’un traumatisme crânien.
D’une infection cérébrale (encéphalite).
D’une tumeur cérébrale.
De certaines maladies neurodégénératives, comme l’aphasie primaire progressive qui peut parfois être le symptôme d’un début atypique de la maladie d’Alzheimer.
Comment évolue ce trouble du langage ?
Ce symptôme, dans les suites d’un AVC, évolue en trois phases :
La phase aigüe : Récente, la lésion provoque de nombreuses modifications dans le cerveau. Le patient nécessite des soins intensifs pour prendre en charge son AVC. Le malade qui présente une aphasie est généralement déstabilisé. Les symptômes initiaux sont parfois sévères, allant jusqu’au mutisme total, c’est-à-dire l’impossibilité de produire un mot. « La rééducation commence dès cette phase », indique le médecin.
La phase subaigüe : Le cerveau du patient se réorganise. De nombreuses zones affectées, mais non lésées se remettent en activité. Une rééducation orthophonique intensive est préconisée, car c’est à cette période que les progrès sont les plus fulgurants. « C’est aussi une phase un peu critique, car le cerveau a subi de nombreuses modifications dont le patient prend conscience. Elles peuvent être à l’origine d’une dépression », ajoute le Dr Bertrand Glize.
La phase chronique : Les mécanismes de neuroplasticité ne sont plus aussi intenses que dans la phase précédente. Les progrès continuent, mais se font plus discrets. La maladie s’intègre à la vie quotidienne.
Quelles sont les personnes à risque ?
En grande majorité, ce sont les personnes âgées qui sont touchées par cette pathologie. L’âge est, en effet, un des facteurs favorisant des accidents vasculaires cérébraux, des tumeurs ou encore des maladies neurodégénératives. Pour autant, toute personne peut devenir aphasique s’il y a une lésion cérébrale. Et cela, quel que soit son âge.
Combien de temps dure l’aphasie ?
D’un individu à un autre, la récupération ne sera pas la même. Cette dernière dépend de nombreux facteurs comme :
La cause, la taille et la localisation de la lésion ;
La présence ou non de troubles associés à l’aphasie ;
Le traitement et la réponse au traitement.
Est-ce une maladie contagieuse ?
L’aphasie est un symptôme, ce n’est pas une pathologie contagieuse.
Qui et quand consulter ?

Quels sont les traitements de l’aphasie ?
En fonction de la cause de l’aphasie, le traitement de cette cause est différent. Si elle est la conséquence d’une tumeur cérébrale par exemple, le traitement associé à cette pathologie est, bien évidemment, préconisé.
Le traitement de ce symptôme repose, quoi qu’il en soit, sur une thérapie du langage spécifique. Principalement sur le rôle des odorants dans le dépistage précoce de la maladie d’Alzheimer, et l’apport de l’olfaction pour ramener à un ressenti émotionnel les patients qui souffrent d’apathie et voient leurs émotions émoussées. L’odorat est le seul sens qui puisse s’expérimenter dans une double voie: il peut y avoir un relais au niveau du cortex orbito-frontal et du cortex limbique (émotionnel), et nous prenons alors conscience de l’odeur ; ou bien le cortex olfactif envoie l’information directement au cerveau limbique sans passer par la voie de la conscience. La composante amygdalienne et donc les émotions peuvent être réactivées dans tous les cas. Si l’on fait sentir l’odeur du pain chaud à un malade d’Alzheimer qui a été boulanger, il ne pourra pas forcément identifier cette odeur mais il y restera particulièrement sensible. On l’observe aussi avec les victimes d’attentat ou les personnes ayant subi un grave traumatisme: les odeurs de sang réveillent la mémoire émotionnelle.
Le vécu individuel compte donc beaucoup ?
Oui, l’odeur d’herbe coupée parlera à certains et pas à d’autres. C’est pour cela que nous ciblons le vécu de la personne que nous cherchons à stimuler ou apaiser. L’origine culturelle est aussi importante: à Taïwan, on ne supporte pas l’odeur de lait chaud mais on raffole des œufs de cent ans et de leur odeur de pourri! La réactivité à certaines odeurs est aussi liée aux habitudes alimentaires. En Inde, le parfum du clou de girofle régale car il est utilisé dans de nombreux desserts, alors que chez nous il évoque le dentiste…
Mais certaines molécules ont des vertus générales…
Le linalol, molécule de la lavande, a des vertus anxiolytiques ; les odeurs «vertes» (pomme, herbe coupée) apaisent… Dans notre laboratoire, nous cherchons des molécules permettant de calmer des états de stress. Pour cela, nous prenons des mesures physiologiques suite à l’exposition aux odorants, telle la réponse électrodermale (en lien avec la sudation). Mais il semble important d’envisager une utilisation à long terme pour constater des améliorations durables sur les aspects comportementaux. Chez les malades d’Alzheimer qui montrent des états d’agitation ou qui ont besoin d’être dynamisés, nous procédons pendant six mois au moins, à leur domicile, à des diffusions d’odorants. Parfois, ces prescriptions sont mal acceptées car les malades ont l’impression de ne rien sentir. Mais l’effet actif des molécules n’en est pas moins certain.
